Here are a couple of tips:

– You may have to open or view as an Adobe file so you can fill it out before printing.  Check the upper right hand of your screen and look for the arrow pointing down.  We prefer you fill it out online, that way we are getting clear information, that is easy to read.  You can save a copy for your records too!  Get Adobe Here

– Make sure to fill out any form COMPLETELY and CAREFULLY!  You are ultimately responsible for correct information going on your report.  Make sure to include any and all tests you want for every sample submitted.  This will ensure a timely turn around for you.

– You are also financially responsible for any and all tests requested on your form, so double check that you are getting what you want!
