Sample Submission Guidelines and
Forms
Submitting a high-quality, properly collected sample is critical for an accurate analysis. Samples that are too small or improperly packaged may delay results or be unsuitable for testing.
You must FULLY complete each form for us to properly process your sample and give recommendations.
If you submit an old form, we will call or email and ask you to submit a completed new form. If we don’t hear from you within two weeks, we will cancel testing and dispose of your sample.
