Request Services
To request services, please follow these steps:
- Step 1: Download and complete one Service Request Form for each service you need, from the Service Request Forms section below
- Step 2: Submit all Service Request Forms using the bottom left link on this page.
- Each service requires its own submission form. Services (including multiple skus) are not to be combined into one form.
- You will not be able to submit a request without the appropriate documentation attached.
- Only attach OUR FORMS. Any additional documents you have can be submitted at a later time when we have started work on your project.
- A member of our team will follow up with you within 5-7 business days of your completed submission.
If your service requires SAMPLE SUBMISSION: EACH sample must be clearly labeled with Product Name, Date, Client Name. You are also required to include a physical copy of your submission form that includes your Contact information and storage instructions.
If samples are received without the proper labeling, they will be discarded after 5 business days; client will be responsible for submitting new samples in order for the requested services to be executed

These request forms are required to be submitted for the NFP, Process Authority Letter, Food Analysis or Shelf-Life Testing.

- If you do not see a form for a service you are requesting, please use the Contact the FIC button to submit a general inquiry.
Non-Disclosure Agreement (NDA)
We do not require this form, but if you would prefer to have an NDA in place in order for us to perform services, please complete this NDA and submit along with your service request forms. Please note NDA’s take a minimum average of two weeks to process, but we can concurrently work on your product while this is in process.
